Wonder if you have CFS or fibromyalgia? If you have had severe fatigue or widespread pain lasting over three months without an obvious cause, and if you also have insomnia, then you might have chronic fatigue syndrome and/or fibromyalgia. The checklist below can help you assess the possibility of this. If your score indicates you might, then you should consult a physician to find out for sure.

The Checklist uses two sets of criteria to evaluate your posibility of having CFS or fibromyalgia. Questions 1-5 are based on criteria established by the CDC (Center for Disease Control and Prevention) for evaluating a person's possibility of having CFS. Questions 6-10 are based on criteria established by the ACR (American College of Rheumatology) for evaluating a person's possibility of having fibromyalgia.

CFS Checklist

1. Do you frequently experience unusually long periods of fatigue?

2. Is your fatigue the result of some obvious ongoing physical exertion that you are aware of?

3. Does your fatigue go away after you've rested normally?

4. As a result of your ongoing fatigue, have you substantially reduced your previous levels of occupational, educational, social, or personal activities?

5. Check each of the following that began at about the same time as your fatigue, and that persisted for at least 6 months.








Fibromyalgia Checklist

6. Check each area you've had pain over the last week.


















7. Rate the severity of your fatigue during the past week.




8. Rate the severity of your having felt unrefreshed when you woke up each morning during the past week.




9. Rate the severity of your cognitive dysfunction (i.e., feelings of "Brain Fog") during the past week.




10. Check each of the following symptoms that you have experienced during the past 6 months.
